Team Achievement Nomination Form

Friday, March 10, 2023, at 5:00 p.m. Nominations received after this time will not be considered. Award recipients will be announced and then recognized during the Staff Service Awards and Recognition Ceremony on Friday, June 9, 2023.

Description:

The Team Achievement Award recognizes a team of staff members who collaborated on a project, process, or other significant initiative to achieve distinctive results/solutions which positively impacted University operations, improve productivity, and/or reduce costs, and benefited a major sector of the University. The team aligned actions and decisions with the University's core values of excellence, integrity, service, creativity, innovation and educational opportunity.

Eligibility:

State-side staff employees only, including MPPs. If a student assistant has a significant role as a part of this team, they may be considered. Faculty are not eligible. Teams should consist of a minimum of three individuals. Work groups, departmental units, or cross-functional teams/groupings are eligible.

    • Service: Exceptional efforts to enhance service to the “customer” in support of the University’s mission.
    • Teamwork: Assessment of the diverse strengths and abilities in a group setting and then applying them to one final solution.
